RESUMO

In the study of liver cancer, invasive techniques such as computed tomography (CT) and magnetic resonance imaging (MRI) are used. In the present work, an in vivo 3D study will be carried out using MRI and CT images, using scaling dynamics, that is, calculating critical exponents of local roughness and fractal dimension. The results obtained for stages IA, IIA, and IIIA in liver cancer correspond to what was proposed by F. Family and T. Vicsek (1991).

RESUMO

A 91-year-old man presented with pruriginous tense blisters and erosions on the upper and lower extremities (Figures 1A and 1B). Mucous membranes were unaffected and Nikolsky's sign was negative. These lesions appeared 48 hours after the administration of the second dose of the Pfizer-BioNTech COVID-19 vaccine. The patient had received the first dose of the same vaccine 23 days prior to the onset of lesions. He did not suffer from any other post-vaccination adverse effects.

RESUMO

Tumor growth in the breast is a complex process, understanding this type of phenomenon is important in order to establish adequate strategies at diagnostic and therapeutic levels, beginning with the evaluation morphology, through adequate descriptors. Scaling analysis is used to extract parameters such as exponents of local roughness and fractal dimension, which characterize tumor growth. The results show that invasive lobular and ductal tumors in situ behave as proposed by Family-Vicsek (Family and Vicsek, 1991), while benign tumors behave differently.

RESUMO

Alopecia neoplastica is a rare type of cutaneous metastasis. The most frequent presentation consists of red-violaceous nodular scarring alopecia located at the parietal area. The most frequent primary tumor locations are the breast and gastrointestinal tract. We report a case of alopecia neoplastica that induced an underlying lytic bone metastasis. After a rigorous literature search, we could not find another case showing this mechanism.

While the importance of physical activity in older adults is beyond doubt, there are significant barriers limiting the access of older adults to physical exercise. Existing technologies to support physical activity in older adults show that, despite their positive impacts on health and well-being, there is in general a lack of engagement due to the existing reluctance to the use of technology. Usefulness and usability are two major factors for user acceptance along with others, such as cost, privacy, equipment and maintenance requirements, support, etc. Nevertheless, the extent to which each factor impacts user acceptance remains unclear. Furthermore, other stakeholders, besides the end users, should be considered in the decision-making process to develop such technologies, including caregivers, therapists and technology providers. In this paper, and in the context of physical rehabilitation and exercise at home, four different alternatives with incremental characteristics have been defined and considered: a software-based platform for physical rehabilitation and exercise (Alternative 1), the same software platform with a conventional RGB camera and no exercise supervision (Alternative 2), the same software platform with a convention RGB camera and exercise supervision (Alternative 3) and finally, the same software platform with a depth camera and exercise supervision (Alternative 4). A multiple attribute decision-making methodology, based on the ordinal priority approach (OPA) method, is then applied using a group of experts, including end users, therapists and developers to rank the best alternative. The attributes considered in this method have been usefulness, cost, ease of use, ease of technical development, ease of maintenance and privacy, concluding that Alternative 3 has been ranked as the most appropriate.

BACKGROUND: Hamstring injury prevention programs include strengthening, especially eccentric exercises using both gravitational and inertial loading. Inertial exercises are characterized by eccentric contractions of high intensity and velocity. This study aimed to analyze the muscular activation of the biceps femoris (BF), semitendinosus (ST), gluteus maximus (GM), and gracilis (GC) muscles during hip extension (HE) exercises performed under both gravitational and inertial loading conditions. HYPOTHESIS: Inertial training would generate a greater activation of HE muscles than gravitational training. STUDY DESIGN: Cross-sectional study. LEVEL OF EVIDENCE: Level 4. METHODS: Fifteen resistance-trained men performed the unilateral straight knee bridge (SKB), 45° of HE, and stiff-leg deadlift (SDL) exercises under gravitational and inertial loading conditions. Concentric and eccentric phases were identified with a linear encoder. Differences between load types, exercises, and their interaction were examined to establish the electromyographic (EMG) activity of each muscle and BF/ST ratio. RESULTS: In the concentric phase, inertial loading showed a higher normalized EMG than gravitational loading for BF, ST, and GM. SKB and HE activated BF and ST between 9.6% and 24.3% more than SDL. In the eccentric phase, the inertial modality achieved greater GM activation than the gravitational form (18.1%). BF activation was increased with HE and SKB as compared with SDL (24.4% and 16.4%, respectively), while ST activation was likewise enhanced with HE as compared with SDL (15.1%). CONCLUSION: Inertial training is more effective than gravitational training for the concentric activation of the hamstring muscles while SDL showed lower hamstring activation than HE and SKB. Therefore, HE and SKB with inertial loading should be taken into account in hamstring training programs. CLINICAL RELEVANCE: Inertial training is more effective than gravitational training for the concentric activation of the hamstring muscles. HE and SKB with inertial loading should be taken into account in hamstring training programs.

Introduction: The COVID-19 outbreak is having an impact on the well-being of healthcare workers. Mindfulness-based interventions have shown effectiveness in reducing stress and fostering resilience and recovery in healthcare workers. There are no studies examining the feasibility of brief mindfulness-based interventions during the COVID-19 outbreak. Materials and Methods: This is an exploratory study with a post intervention assessment. We describe an on-site brief mindfulness intervention and evaluate its helpfulness, safety, and feasibility. Results: One thousand out of 7,000 (14%) healthcare workers from La Paz University Hospital in Madrid (Spain) participated in at least one session. One hundred and fifty out of 1,000 (15%) participants filled out a self-report questionnaire evaluating the helpfulness of the intervention for on-site stress reduction. Ninety two subjects (61%) participated in more than one session. Most of the participants were women (80%) with a mean age of 38.6 years. Almost half of the sample were nurses (46%). Sessions were perceived as being helpful with a mean rating of 8.4 on a scale from 0 to 10. Only 3 people (2%) reported a minor adverse effect (increased anxiety or dizziness). Discussion: Our data supports the utility, safety and feasibility of an on-site, brief mindfulness-based intervention designed to reduce stress for frontline health workers during a crisis. There is a need to continue testing this type of interventions, and to integrate emotion regulation strategies as an essential part of health workers' general training. BACKGROUND: Cardiac surgery-related acute kidney injury (AKI) is a common postoperative complication that greatly increases morbidity and mortality. There are currently no effective interventions to prevent AKI associated with cardiac surgery. Experimental data have shown that administration of the mineralocorticoid receptor blocker spironolactone prevents renal injury induced by ischemia-reperfusion in rats. The objective of this study was to test whether short-term perioperative administration of oral spironolactone could reduce the incidence of AKI in cardiac surgical patients. STUDY DESIGN: Randomized, double-blinded, placebo-controlled trial. SETTING & PARTICIPANTS: Data were collected from April 2014 through July 2015 at the National Heart Institute in Mexico. 233 patients were included; 115 and 118 received spironolactone or placebo, respectively. INTERVENTION: Spironolactone or placebo once at a dose of 100mg 12 to 24 hours before surgery and subsequently 3 further doses of 25mg in postoperative days 0, 1, and 2 were administered. OUTCOMES: Patients were followed up for 7 days or until discharge from the intensive care unit (ICU). The primary end point was AKI incidence defined by KDIGO criteria. Secondary end points included requirement of renal replacement therapy, ICU length of stay, and ICU mortality. Data were analyzed according to the intention-to-treat principle. RESULTS: Mean age was 53.2±15 years, mean serum creatinine level was 0.9±0.2mg/dL, median Thakar score for estimation of AKI risk was 2 (IQR, 1-3), and 25% had diabetes. The incidence of AKI was higher for the spironolactone group (43% vs 29%; P=0.02). No significant differences were found for secondary end points. LIMITATIONS: Single center, AKI was mostly driven by AKI stage 1, planned sample size was not achieved, and there was no renin-angiotensin-aldosterone system washout period. CONCLUSIONS: Our trial demonstrated that spironolactone was not protective for AKI associated with cardiac surgery and there may be a trend toward risk.

RESUMO

Renal cell adenocarcinoma requires different therapeutic pathways because it is one of the most therapy-resistant tumors, on the other hand it is biologically one of the most attractive tumors. Its pathological classification has a genetic base. There is an anomaly of the Von Hippel Lindau gene in 80% of adenocarcinomas, being this fact determinant to know the biological characteristics of tumor initiation and development, as well as the identification of factors susceptible to be used as therapeutic targets. Since 2005 a group of molecules have been used in the treatment of metastatic adenocarcinomas and, even though therapeutic results are significant but not clinically relevant yet, we are sure they are a key way for more efficient future developments. The present study tries to make a tour on the research of the biological anomalies in renal adenocarcinoma with special emphasis in the Von HippelLindau gene.
